DISCUSSION:
The County of San Bernardino Community Development & Housing Department awarded the City of Fontana $500,000 in program assistance through the Housing and Homelessness Incentive Program (HHIP).
The Housing and Homelessness Incentive Program (HHIP) is a statewide voluntary incentive program administered by the California Department of Health Care Services (DHCS) that enables health plans to earn one-time incentive funds for improving health outcomes and access to whole person care services by addressing homelessness and housing insecurity as social drivers of health and health disparities. Molina Healthcare of California has allocated HHIP funds to San Bernardino County to fund eligible activities that prioritize gaps in local shelter infrastructure with an emphasis in funding the startup and continued operation of large-scale navigation centers.
The City of Fontana owns The Path, an Interim Housing Facility with 120 beds dedicated to serving individuals and families experiencing or at risk of homelessness. Operated by Symba Center, the Facility utilizes a medically based model of homeless care, employing a multi-disciplinary team and evidence-based practices to effectively meet the needs of its residents. To ensure the continued provision of critical services and operations, the City intends to use this two-year grant to support and sustain the Facility’s programs, offering ongoing assistance to those in need.
