RECOMMENDATION:
Recommendation
1. Approve a proposal and authorize the City Manager to enter into an agreement with Helping Hearts of California, in the amount of $498,508.67 for Interim Housing Support Services Operator and Case Management Services at Path located at 17133 Valley Boulevard, Fontana, CA.
2. Approve and authorize the City Manager, or City Manager’s designee, to execute all future amendments to the agreement with Helping Hearts of California for Interim Housing Support Services Operator and Case Management Services.

COUNCIL GOALS:
• Provide a diverse range of housing types and levels of affordability while addressing homelessness in the community by address Homelessness through proactive programs, including the efforts of the COAST and MET teams.
DISCUSSION:
The City Council has determined homelessness and homeless prevention as a top priority and has previously instructed staff to take all feasible actions in support of the City’s homelessness prevention plan.
In March of this year, the City issued Request for Proposals (RFP) HD-26-160-SP for Interim Housing Support Services Operator and Case Management Services to provide Interim Housing Operations and Wellness Services for the Path. The facility will operate as interim housing and provide 24-hour care and monitoring seven days a week, 365 days a year, offering safe shelter, basic needs, and supportive services to individuals experiencing homelessness.
The Request for Proposals (RFP) closed on April 15, 2026. The City received three (3) proposals for case management services, with proposed costs ranging from $443,654 to $671,690 for one year of services. Following a review and evaluation of the proposals, Helping Hearts of California (HHCA) was determined by the evaluation committee to be the highest-scoring responsive proposer, with a proposed cost of $498,509 for one year of services.
Helping Hearts of California, LLC is a California LLC incorporated in the State of California providing services since 2015. HHCA’s mission is to improve the quality of life, health, skills, and stability of those living with mental illness, substance use disorders, medical challenges, unstable housing, and justice involvement. Their proposal includes staffing and operational support for a case management team that will provide direct services at the Path facility.
Services will include case management, Intake coordination, universal assessment, housing navigation, transportation assistance, peer support, problem solving and diversion and HMIS and coordinated entry.
The proposed service model includes dedicated case managers and peer support staff who will provide individualized support to residents while coordinating with regional service providers and the County’s Continuum of Care system. These services will assist clients in addressing barriers to housing stability and transitioning into permanent housing solutions.
City staff is satisfied, based on its review and investigation, that Helping Hearts of California demonstrates the competence, experience, and qualifications necessary for satisfactory performance of the professional services required at a fair and reasonable cost.
FISCAL IMPACT:
The fiscal impact associated with the approval of this agreement is $498,509 and it is included in The Path Operating Expenses 10130105. 8115 FY 2026-2027.
