RECOMMENDATION:

Recommendation

1.                     Approve an Agreement for Grant Funds between the Fontana Housing Authority and Jamboree Housing to facilitate delivery of on-site medical services and programs at the Sierra Fountains apartment project located at 16839 Ramona Avenue.

 

2.                     Authorize the Executive Director to execute any documents necessary or appropriate to effectuate said approval and/or agreement.

COUNCIL GOALS:

                     To concentrate on inter-governmental relations by establishing partnerships and positive working relationships with other public agencies providing services to residents.

                     To promote affordable housing by construction of high-quality multi-family housing which also serves to address the affordability needs of this community.

DISCUSSION:

In February 2018 the Fontana Housing Authority approved a Disposition and Development Agreement (DDA) with Jamboree Housing to facilitate construction of the Sierra Fountains apartment project. Located on the South-West corner of Sierra and Ramona Avenues, directly adjacent to Juniper Elementary School, the sixty (60) unit apartment project includes a significant on-site social services component. Jamboree Housing and Cucamonga Valley Medical Center (CVMC) will be working together to provide on-site medical services and programs.

 

CVMC was originally intending to fully fund the required on-site tenant improvements required to deliver medical related services and programs at the Sierra Fountains apartments. However, due to the ongoing covid pandemic, CVMC has been severely impacted economically. As a result, funding previously earmarked for the required tenant improvements is no longer available. 

 

The Fontana Housing Authority was recently approached by Jamboree Housing and CVMC to provide funding for the tenant improvements required to provide medical related services at the Sierra Fountains apartments. To that end, the Fontana Housing Authority and Jamboree Housing have negotiated a funding agreement with the following proposed deal-points;

 

 

                     The Fontana Housing Authority would provide $250,000 in funding to Jamboree Housing specifically for the purpose of completing any/all tenant improvements required to provide medical services and programs at the Sierra Fountains apartments.

 

                     The funding to Jamboree Housing will be provided as a grant, which is forgiven over a ten-year period. Each year that CVMC provides medical services and programs at the Sierra Fountains apartments the grant will be forgiven by 1/10th.

                     Jamboree Housing will ensure that CVMC completes any/all required tenant improvements to the 3,000 square foot space specifically dedicated for medical related services at the Sierra Fountains apartments.

                     Jamboree Housing will be responsible for ensuring that medical related services and programs are delivered at the Sierra Fountains apartments (by CVMC or a similar provider) for a period of not less than ten (10) years. Said services and programs will include diabetes testing and education, smoking addiction clinics, covid testing and vaccines, breast cancer awareness and testing, health fairs and nutrition education programs.

Approval of the Agreement for Grant Funds with Jamboree Housing will ensure that the required tenant improvements are completed at the Sierra Fountains, allowing for the on-site delivery of medical related services and programs for a period of at least ten (10) years.

 

FISCAL IMPACT:

Funding in the amount of $250,000 is available from the fiscal year 2021-22 Housing Authority budget in the Ramona Multifamily Apts Project #20203864-297-A.
